Continuing Care Coordinator

Job Summary: Nova Scotia Health is seeking a Care Coordinator to work in a community-based setting, providing assessment, care planning, and case management services to clients referred to Continuing Care.
The Care Coordinator will be responsible for assessing clients' needs, developing care plans, and authorizing services to support clients in their homes and communities. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a multidisciplinary team.
About the Role: As a Care Coordinator, you will work closely with clients, families, caregivers, and healthcare teams to identify clients' goals, unmet needs, and resource requirements. You will develop and implement care plans that address these needs, and work with clients and their care partners to achieve their goals.
Key Responsibilities:
- Assess clients' needs and develop care plans that address these needs
- Authorize services and make referrals to other professional and volunteer resources
- Work with clients and their care partners to achieve their goals
- Collaborate with healthcare teams to ensure seamless care transitions
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
Requirements:
- Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
- Current and ongoing registration with the applicable Nova Scotia Professional Regulatory body
- Minimum three years' experience with the professional regulatory body
- Preference given to candidates with more than five years' experience or those who have experience working in the community
- Experience working in a team-based care setting is considered an asset
Working Conditions: This is a permanent full-time position, working 70 hours bi-weekly. Shifts may include days, evenings, weekends, and holidays.
Compensation: $41.06 - $46.75 hourly. Successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
