Client Care Coordinator

We are a leading home health care provider, committed to delivering exceptional patient care and support. As a Client Care Coordinator, you will play a vital role in our team, working collaboratively with community healthcare professionals to provide high-quality care to our clients.
Key Responsibilities
- Work closely with community Personal Support Workers (PSWs) and Nurses to coordinate and schedule services
- Liaise with clients, staff, and community partners to ensure seamless care delivery
- Act as a problem solver for clients, responding to inquiries and concerns from the Local Health Integration Network (LHIN) and employees
- Report client and staff concerns to the Home Support Supervisor
- Liaise with LHIN representatives to ensure optimal client care
- Collect and enter client data into our computer system
- Provide administrative support, including scheduling, lists, and other duties to support the PSW and Nurse program
Requirements
- Minimum 2 years of experience in an office setting
- Excellent customer service and communication skills, with the ability to handle high volumes of calls
- Ability to multitask and prioritize tasks
- High school diploma or equivalent
- Proficient in Microsoft Office, including Word and Outlook
- Medical terminology knowledge an asset
