Forensic Psychiatric Services Outreach Worker

Job Summary
We are seeking a highly skilled and compassionate Outreach Worker to join our Forensic Psychiatric Services team. As an Outreach Worker, you will play a vital role in providing support and services to individuals with complex mental health and substance use challenges.
Key Responsibilities
- Monitor, screen, and interview clients to obtain information regarding their health condition and environment.
- Report significant and unsafe behavior, physical and cognitive changes, in accordance with standard procedures.
- Arrange, plan, escort, and accompany clients on medical appointments and errands.
- Create linkages to other community resources and programs.
- Act as a liaison between various agencies and provide information on services.
Requirements
- Graduation from a recognized Social Services Worker certificate program or an equivalent combination of education, training, and experience.
- Valid Class 5 vehicle license.
- Awareness of and commitment to learning and understanding the Truth & Reconciliation Commission's 94 Calls to Action.
