Forensic Psychiatric Services Outreach Worker

Job Summary
We are seeking an experienced Outreach Worker to join our Forensic Psychiatric Services team. The successful candidate will work with clients who have complex mental health and substance use challenges, providing support and guidance to help them achieve their goals.
Key Responsibilities
- Monitor and screen clients to identify their health needs and develop individualized plans to support their recovery.
- Provide emotional support and guidance to clients, helping them to manage their mental health and substance use challenges.
- Link clients with community resources and services, such as housing, employment, and education.
- Act as a liaison between clients, family members, and healthcare providers, ensuring that everyone is informed and involved in the client's care.
- Participate in multidisciplinary teams to develop and implement care plans that meet the unique needs of each client.
Requirements
- Degree in Social Work or a related field.
- Experience working with clients who have complex mental health and substance use challenges.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Valid Class 5 vehicle license.
