Forensic Community Liaison Nurse

Job Summary
We are seeking a highly skilled Forensic Community Liaison Nurse to join our team at PHSA. As a key member of our team, you will be responsible for providing high-quality nursing care to individuals with complex mental health and substance use challenges.
Key Responsibilities
- Conduct comprehensive biopsychosocial assessments and develop integrated treatment plans for clients.
- Provide community case coordination and advocacy services to ensure continuity of care and sharing of pertinent information.
- Maintain accurate and concise documentation of client information in accordance with policy standards.
- Provide nursing services, information, and support to clients and their families.
Requirements
- Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse or Registered Psychiatric Nurse.
- Two years recent, related experience working with individuals in the community with severe and persistent psychiatric illness.
- Valid BC Drivers License.
