Job Summary

Fraser Health is seeking a skilled Outreach Worker to join our Overdose Outreach Team. As a member of our interdisciplinary team, you will provide support services to individuals and their families with accessing opioid agonist therapy and related support services. Your role will involve monitoring clients, arranging appointments, and providing emotional support and physical assistance to further the development of self-management skills.

Responsibilities

  1. Monitor clients by observing their health condition and environment, documenting behavioral, physical, and cognitive changes, and reporting significant and unsafe observations to the Registered Nurse or delegate.
  2. Arrange, plan, escort, and accompany clients on medical appointments and errands, providing emotional, moral support, and physical assistance to further the development of self-management skills.
  3. Create linkages to other community resources and programs, contact relevant organizations, and assist clients with processing forms.
  4. Assist clients in accessing benefits or assistance as appropriate and provide brief assessment and planning on issues related to substance use, opioid agonist therapy, overdose prevention, and harm reduction.
  5. Administer medication to clients under the direction of the Registered Nurse and provide medication reminders.
  6. Act as a liaison between various agencies, updating designated team staff on services currently available and liaising with community agencies to provide information on services.
  7. Identify client needs for services and make appropriate referrals or develop alternate strategies for client support, education, and follow-up care.
  8. Provide basic health-promotion information to clients, promoting development of life-skills and encouraging client empowerment and capacity to exercise choice and actively participate in decisions affecting their own health and healthcare.
  9. Perform a variety of general clerical duties, attend meetings, and participate in patient care planning and coordination.
  10. Maintain accurate records of outreach activities and provide regular reports.
  11. Carry out responsibilities in accordance with health and safety requirements.
Qualifications

Education and Experience

Completion of Substance Use Certificate plus one year's recent related experience working with clients with substance use issues, or an equivalent combination of education, training, and experience.

Valid BC driver's license and vehicle.

Current CPR certificate.
