Job Title: Community Liaison Specialist

About the Role

Vancouver Coastal Health is seeking a highly skilled Community Liaison Specialist to join our team in Sechelt, BC. As a member of our Intensive Case Management Team, you will play a vital role in supporting clients with mental health and substance use issues.

Key Responsibilities:
  • Support an integrated health service team and delivery model by providing support services to clients with complex needs.
  • Work collaboratively with interdisciplinary care teams, including primary care, mental health, and substance use professionals, to ensure comprehensive care for clients.
  • Monitor client progress, document observations, and provide verbal reports as required.
  • Facilitate access to community services and resources, and provide information on available options.
  • Liaise and coordinate services with mental health, substance use, and medical professionals to ensure seamless care.
  • Plan, organize, facilitate, and co-lead groups as required, and participate in case planning and coordination.
About You

We are looking for a candidate with a strong background in community health and a passion for working with vulnerable populations. Your excellent communication and interpersonal skills will enable you to establish and maintain rapport with clients, colleagues, and community partners.

Requirements:
  • Certificate from a recognized health care worker program, such as a Community Health Worker Program.
  • Two years' recent experience supporting clients through a holistic mental health, substance use, and primary care lens, or an equivalent combination of education and experience.
  • Valid driver's license and access to a personal vehicle for business purposes.
