Mental Health Community Liaison Specialist

About the Role:
The Vancouver Coastal Health is seeking a qualified Community Liaison Worker to join our Intensive Case Management Team in Sunshine Coast Mental Health in Sechelt, BC.
As a Community Liaison Worker, you will provide support services to clients with mental health and substance use issues, working across multiple sites, clinics, and services within an integrated health service delivery model.
You will be responsible for monitoring clients, documenting observations, and providing verbal reports in accordance with standard procedures or specific direction. You will also facilitate the use of community services/resources, provide information on community services, and liaise and coordinate services with mental health and substance use professionals.
Key Responsibilities:
- Support an integrated health service team and delivery model
- Provide support services to clients with mental health and substance use issues
- Monitor clients, document and provide verbal reports on observations

Requirements:
To be considered for this role, you must have a Certificate in a recognized Health Care Worker program, such as a Community Health Worker Program. You must also have two (2) years' recent related experience supporting clients through a holistic mental health, substance use, and primary care lens, or an equivalent combination of education, training, and experience.
