Job Summary

Vancouver Coastal Health is seeking a skilled Community Liaison Worker to join its Complex Care Housing Team in Sechelt, BC. The ideal candidate will possess a graduate degree in a relevant field and have experience supporting clients with mental health and substance use issues.

As a Community Liaison Worker, you will work closely with an integrated health service team to provide support services to clients. Your responsibilities will include monitoring client progress, documenting observations, and facilitating access to community resources.

You will also liaise with mental health and substance use professionals to ensure comprehensive care. Strong communication and interpersonal skills are essential for this role, as you will work with clients, families, and healthcare professionals to achieve positive outcomes.

Key Responsibilities:

  1. Support an integrated health service team and delivery model
  2. Provide support services to clients with mental health and substance use issues
  3. Monitor client progress and document observations
  4. Facilitate access to community resources
  5. Liaise with mental health and substance use professionals

Requirements:

  • Graduate degree in a relevant field
  • 2 years' recent experience supporting clients with mental health and substance use issues
  • Valid B.C. Driver's license and access to a personal vehicle for business purposes
