Job Summary:
We are seeking a dedicated and experienced Clinical Lead Nurse to join our healthcare team at Madison Village Long Term Care. The successful candidate will be responsible for overseeing and coordinating patient care delivery, providing clinical guidance to nursing staff, and ensuring adherence to healthcare regulations and protocols.
The Clinical Lead Nurse will work under the direction of the Clinical Manager (CM) and oversee nursing care practices to residents in accordance with required medical care and established nursing care plans and practices. This includes assessment, data collection, and tracking of mandatory programs and other incidents related to resident care and safety.
Responsibilities include receiving shift report from the Charge Nurse upon arrival, taking over the primary supervision of assigned unit/area, and following up on any concerns within the past 24 hours. This may include pain management, falls prevention, infection control, wound care, behavioral issues, resident transfers to hospital, returns from hospital or extended leaves, outstanding lab reports, and significant changes in residents' condition.
The Clinical Lead Nurse is also responsible for ensuring that care plans are developed, updated as needed, and reviewed on a quarterly basis as per established schedule. This involves using RAI MDS, including coding, RAPS, in accordance with MOLTC legislated requirements.
Superior communication and interpersonal skills are essential for this role, as the Clinical Lead Nurse will supervise and provide direction to Registered Practical Nurses (RPNs) and Personal Support Workers (PSWs), ensuring that resident care is provided in accordance with care plans and Home's policies and procedures.
Qualifications:
- Valid RN/RPN license
- Current registration with College of Nurses of Ontario
- Long-Term Care experience
- Strong knowledge of geriatrics, LTC, and FLTCA, 2021, and associated Regulations
- Ability to work effectively in a fast-paced environment
- Excellent communication and interpersonal skills
- Compassionate attitude towards patient care
