Job Overview

A unique opportunity has arisen for an experienced Registered Nurse to join our dynamic team at Hamilton Health Sciences Corporation in Ontario-Hamilton. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and leading the coordination and evaluation of nursing care for patients with complex medical conditions.

This is a regular part-time position, requiring a rotating schedule that includes days, evenings, nights, weekends, and holidays. You must have excellent leadership, critical thinking, decision-making, communication, organizational, problem identification, and problem-solving skills to excel in this role.

About the Position

This exciting opportunity is located at the Hamilton General Hospital, one of Canada's largest and most diverse hospitals. As a Short Stay Medicine Unit (SSMU) nurse, you will work collaboratively with an interdisciplinary team to provide high-quality patient care, focusing on geriatric patients with multi-system chronic illnesses. Your primary goal will be to ensure seamless transitions to community settings.

Responsibilities
  • Lead the coordination and evaluation of nursing care for patients with complex medical conditions
  • Collaborate with an interdisciplinary team to develop and implement patient-centered care plans
  • Provide education and support to patients and their families
  • Ensure timely discharges within 72 hours or less
  • Participate in ongoing learning and professional development

Requirements

To be considered for this opportunity, you must possess:

  • Current Registration with the College of Nurses of Ontario as a Registered Nurse
  • Current Basic Cardiac Life Support (BCLS) certification
  • Experience in a medical setting, preferably with telemetry experience
  • Knowledge of current nursing assessment and management of adult and geriatric medicine patients
  • Demonstrated leadership and critical thinking skills
