Community Health Physicist

We are seeking a highly skilled and compassionate Community Health Physicist to join our team at the Hamilton Family Health Team. As a member of our healthcare team, you will work closely with patients and other healthcare professionals to provide high-quality care and improve health outcomes.
The successful candidate will be responsible for assessing and treating individuals who self-refer or are referred by our physicians. This includes diagnosing within their scope of practice and establishing evidence-informed treatment plans that integrate with existing community services.
A strong knowledge of community-based programs and services is essential to facilitate clients' access to external services and manage caseload demands. The ideal candidate will possess primary care experience, preferably with a supportive approach to patients struggling with social determinants of health.
In this role, you will participate in and lead group programs, such as chronic pain self-management and osteoarthritis exercise programs. A background in pelvic health physiotherapy is preferred. Additional responsibilities include:
- Assessing and treating patients with complex conditions, including chronic pain
- Developing self-management skills and promoting patient independence
- Facilitating group education sessions and population health interventions
- Collaborating with inter-professional teams and building effective relationships with patients
Salary and Benefits
- Competitive salary: $75,000 - $95,000 per year (based on location and experience)
