Community Health Nurse Specialist

As a Nurse in our Community Care program, you will have the opportunity to make a meaningful difference in the lives of our clients. Your responsibilities will include:
- Conducting comprehensive client assessments to develop personalized care plans
- Implementing and evaluating care based on best practices and client needs
- Collaborating with the care team to support clients through care transitions
- Connecting clients with community support services in their local area

Canadian Nurses Association (CNA) specialty certification is an asset, and we encourage you to pursue your interests and passions in community care and evidence-informed practice. A valid driver's license and vehicle are required for travel, and availability to work days Monday-Friday is necessary.
