Part-Time Community Care Nurse

We are currently hiring a Part-Time Registered Practical Nurse (RPN) and Part-Time Registered Nurse (RN) to join our team at SE Health. Our nursing team provides supportive care with flexible schedules, paid training, and competitive pay. As a nurse with SE Health, you will have the opportunity to work in a variety of patient-facing nursing roles in different locations or pivot to supporting or leadership positions where you use your nursing skills as a foundation to contribute and make an impact in different ways. Some of the key responsibilities of this role include:
- Complete comprehensive client assessments and develop care plans
- Implement and evaluate care based on best evidence/practices and on client's unique needs
- Support the client through care transitions by collaborating and connecting the client with community support services in their local community

The ideal candidate will have a valid driver's license and vehicle (travel required) and be able to commit to a 24-month employment period at a full-time work schedule offered by the employer.
