Compassionate Community Care Nurse

We are currently hiring Part-Time Registered Practical Nurses (RPNs) and Part-Time Registered Nurses (RNs) to join our dynamic team at SE Health. Our organization is dedicated to providing exceptional care and services to clients across Canada.
About the RoleThis Nursing position is part of the Community Commitment Program for Nurses (CCPN), which offers a $25,000 incentive to eligible nurses in exchange for a 24-month commitment to an eligible employer.
As a compassionate Community Care Nurse, you will be responsible for providing high-quality care to clients in their homes or communities. You will work collaboratively with other healthcare professionals to develop and implement personalized care plans that meet the unique needs of each client.
Key Responsibilities- Complete comprehensive client assessments to identify individualized care needs
- Develop and implement care plans based on best evidence and practices
- Support clients through care transitions by connecting them with community resources and services
You will also have the opportunity to work in various nursing roles within SE Health, including patient-facing positions and supporting or leadership roles.
Requirements- Valid driver's license and vehicle (travel required)
- Availability to work days, 9:00am-5:00pm and 3:00pm-8:00pm, and every other weekend
- Canadian Nurses Association (CNA) specialty certification an asset
