About the Role

The Indigenous Transitions Navigator or bami'iwe position will support the organization in its commitment to reconciliation. The role has been developed from guidance from the community and local Indigenous partnerships.

Job Description

This is a regular full-time position located in Chatham-Kent, Ontario, within the Equity, Diversity, Inclusion and Anti-Racism department.

About You

To be successful in this role, you will have a minimum of five years' work experience in an Indigenous health and or social service organization, hospital, or community-based case management role. You will also have knowledge of biopsychosocial assessment models and social determinants of health. A nursing or social work/social service background is preferred. Experience working with First Nations community and urban indigenous community is essential. Preference will be given to an Indigenous person from a local indigenous community.

What We Offer

We offer a competitive salary range between $80,000-$100,000 annually, depending on experience, plus benefits. This role offers opportunities for growth and development in a dynamic and supportive environment.
