About the Role

WoodGreen is seeking a highly skilled and compassionate Crisis Specialist to join our team. As a Crisis Specialist, you will play a critical role in providing crisis intervention and support services to seniors with complex needs.

Key Responsibilities
  • Participate in crisis shifts and visit clients in their homes or community locations with a partner.
  • Perform psychosocial and geriatric assessments to identify client needs and develop effective care plans.
  • Provide short-term intensive case management services to a caseload of seniors with mental health and substance use issues.
  • Assess risk in crisis situations and develop intervention and action plans to ensure client safety.
  • Coordinate services and care plans with community professionals and agencies to ensure a continuum of care.
  • Provide supportive counseling and advocacy services to seniors and their families.
  • Work collaboratively with the hospitals and other referral sources to support clients and facilitate smooth transitions between services.
  • Engage in ongoing outreach and community engagement to ensure access to the program.
  • Maintain and develop contacts with other seniors' groups and community agencies.
  • Provide mentorship, training, and supervision to student placements and volunteers.
Requirements
  • Masters of Social Work or post-graduate degree in a relevant field with 2+ years of experience in crisis support and complex seniors' care.
  • Registration in good standing with the Ontario College of Social Workers and Social Service Workers or another relevant regulatory body in Ontario.
  • Experience in crisis support and intervention, with a focus on working with seniors with complex needs.
  • Strong assessment, clinical, and intervention skills, with a client-centered approach.
  • Effective oral, written communication, documentation, interpersonal, time management, and organizational skills.
  • Ability to work in a multi-disciplinary team and collaborate with community professionals.
  • Knowledge of theories of addiction, evidence-based treatment models, and best practice approaches.
  • Thorough knowledge of issues affecting older adults and geriatric populations, including mental health, addictions, and health sector resources.
