About the Role

WoodGreen Community Services is seeking a skilled Crisis Specialist to join our team. As a Crisis Specialist, you will play a critical role in providing support and services to seniors with mental health and substance use issues.

Key Responsibilities
  • Participate in crisis shifts and visit clients in their homes or community locations with a partner.
  • Perform psychosocial and geriatric assessments to identify client needs and develop effective care plans.
  • Provide short-term intensive case management services, including ongoing assessment, service navigation, and care coordination.
  • Assess risk in crisis situations and develop intervention and action plans to ensure client safety.
  • Provide service/care coordination, system navigation, short and long-term planning, and follow-up support to clients and their families.
  • Develop and implement care plans to address client priority and emerging needs, in collaboration with clients and their families.
  • Provide supportive counseling to seniors and older adults using a range of therapeutic approaches.
  • Participate in case conferences and clinical meetings to ensure coordination of services and a continuum of care.
  • Advocate for clients and their families, and provide support and guidance to help them navigate the healthcare system.
  • Work closely with hospitals and other referral sources to support clients and facilitate smooth transitions between hospitals and community services.
  • Provide services that are responsive, culturally sensitive, and linguistically accessible to clients and their families.
  • Engage in ongoing outreach to ensure access to the program and maintain relationships with community agencies and seniors' groups.
  • Maintain accurate records and documentation, and collect statistical data required for program accountability.
Requirements
  • Masters of Social Work or post-graduate degree in a relevant field, or BSW with 5+ years of experience in crisis support and complex seniors' care.
  • Registration in good standing with the Ontario College of Social Workers and Social Service Workers or another relevant regulatory body in Ontario.
  • Experience in crisis support and demonstrated experience working with complex seniors, including those with mental health and substance use issues.
  • Ability to work in a multi-disciplinary team and collaborate with community professionals.
  • Knowledge of theories of addiction, evidence-based treatment models, and best practice approaches.
  • Strong assessment, clinical, and intervention skills, working from a client-centered approach.
  • Effective oral, written communication, documentation, interpersonal, time management, and organizational skills.
  • Ability to practice anti-oppression, anti-racism, trauma-informed, and harm reduction principles.
  • Ability to be self-directed, independent, flexible, and exercise good judgment.
  • Ability to minimize barriers through service delivery for low-income individuals with mental health and substance use issues.
  • Knowledge of issues affecting marginalized communities, policies, legislation, programs, and other issues related to scope of practice and social determinants of health.
  • Applied computer skills and ability to use client information systems.
  • Ability to communicate in a second language is a strong asset.
