Program Overview – Crisis Outreach Service for Seniors, Community Care
This role is integral to WoodGreen's collaborative Crisis Outreach Service for Seniors (COSS), aimed at assisting seniors facing physical, cognitive, mental health, and other challenges that may lead to crises. This initiative is a partnership involving WoodGreen, St. Michaels Homes, South Riverdale Community Health Centre, Reconnect Community Health Services, and LOFT Community Services, focused on providing a seamless continuum of care for seniors dealing with cognitive, mental health, and substance use issues.

Key Responsibilities
  • Engage in crisis response shifts; conduct home or community visits to clients alongside a partner.
  • Execute psychosocial and geriatric evaluations.
  • Deliver short-term intensive case management services to a caseload of seniors facing mental health and substance use challenges, including ongoing assessments, service navigation, and care coordination.
  • Assess risk in crisis scenarios, consult, and formulate intervention and action plans.
  • Facilitate service/care coordination, system navigation, and both short and long-term planning and follow-up support.
  • Collaborate with clients to create and implement care plans addressing their immediate and evolving needs.
  • Provide supportive counseling to seniors and older adults using a variety of therapeutic approaches tailored to meet clients' unique needs and circumstances.
  • Participate in case conferences and clinical meetings to ensure coordinated services and continuity of care.
  • Advocate for clients and their families or caregivers.
  • Collaborate closely with hospitals and other referral sources to support clients and ensure smooth transitions between facilities and community services.
  • Deliver culturally and linguistically sensitive services.
  • Engage in ongoing outreach to promote access to the program.
  • Maintain and foster relationships with other senior groups and community organizations, attending external meetings as directed by the supervisor or manager.
  • Stay informed about the range of services available through other agencies.
  • Provide mentorship, training, and/or supervision to student placements and volunteers.
  • Work flexible hours based on client and caregiver needs.
  • Contribute to a positive team environment and participate in regular meetings.
  • Engage in continuous training and education.
  • Participate in supervision sessions and annual performance evaluations with program supervisor/manager.
  • Maintain accurate records and documentation promptly and collect statistical data for program accountability.
  • Ensure the anti-racism objectives and community development principles of the organization are upheld within the program.
Qualifications
  • Master’s degree in Social Work or a relevant post-graduate degree with 2+ years of extensive experience in crisis management with seniors, or a Bachelor of Social Work with 5+ years of relevant experience.
  • Registration in good standing with the Ontario College of Social Workers and Social Service Workers or another relevant regulatory body in Ontario.
  • Proven experience in crisis support and working with complex senior populations.
Preferred Skills
  • Experience in crisis intervention and support.
  • Ability to assist older adults and seniors, including those with significant mental health and/or addiction issues, trauma histories, dementia, cognitive impairments, and chronic health conditions.
  • Proficiency in case management, service planning, implementation, and coordination, as well as therapeutic or supportive counseling.
  • Capability to work within a multi-disciplinary team and collaborate with community professionals.
  • Knowledge of addiction theories, evidence-based treatment models, and best practice approaches.
  • Comprehensive understanding of issues affecting older adults and geriatric populations, along with mental health, addictions, and community resources.
  • Strong assessment, clinical, and intervention skills, utilizing a client-centered approach.
  • Excellent oral and written communication, documentation, interpersonal, time management, and organizational skills.
  • Commitment to anti-oppression, anti-racism, trauma-informed care, and harm reduction principles.
  • Ability to work independently, flexibly, and exercise sound judgment.
  • Skills to reduce barriers in service delivery for low-income individuals with mental health and substance use challenges.
  • Knowledge of issues affecting marginalized communities and relevant policies and legislation.
  • Proficient computer skills (MS Word, Excel, Internet) and ability to utilize client information systems.
  • Proficiency in a second language is a strong asset.
