Program Overview – Crisis Outreach Service for Seniors, Community Care
This role is dedicated to supporting WoodGreen's inter-agency, multi-disciplinary Crisis Outreach Service for Seniors (COSS), which assists older adults facing physical, cognitive, mental health, and other crises. This initiative is a collaborative effort involving WoodGreen, St. Michaels Homes, South Riverdale Community Health Centre, Reconnect Community Health Services, and LOFT Community Services, aiming to provide a seamless continuum of care for seniors experiencing cognitive, mental health, and substance use challenges.

Key Responsibilities
  • Engage in crisis response shifts; conduct home or community visits with a partner.
  • Execute psychosocial and geriatric evaluations.
  • Deliver short-term intensive case management for a caseload of seniors with mental health and substance use challenges, including ongoing assessment, service navigation, and care coordination.
  • Assess risk in crisis scenarios, consult, and formulate intervention and action plans.
  • Provide service coordination, system navigation, and both short and long-term planning and follow-up support.
  • Collaborate with clients to create and implement care plans addressing their immediate and evolving needs.
  • Offer supportive counseling to seniors and older adults using a variety of therapeutic approaches tailored to their unique circumstances.
  • Participate in case conferences and clinical meetings to ensure coordinated services and a comprehensive care continuum.
  • Advocate for clients and their families or caregivers.
  • Collaborate closely with hospitals and other referral sources to facilitate smooth transitions for clients.
  • Deliver culturally and linguistically sensitive services.
  • Engage in ongoing outreach to promote access to the program.
  • Maintain and cultivate relationships with other senior groups and community organizations, attending external meetings as recommended.
  • Stay informed about the range of services offered by other agencies.
  • Provide mentorship, training, and/or supervision to student placements and volunteers.
  • Work flexible hours to meet the needs of clients and their caregivers.
  • Foster a positive team environment and participate in regular meetings.
  • Engage in continuous training and professional development.
  • Participate in supervision sessions and annual performance evaluations with program supervisors/managers.
  • Maintain accurate records and documentation promptly and collect statistical data for program accountability.
  • Ensure the anti-racism objectives and community development principles of the organization are upheld within the program.
Qualifications
  • Masters of Social Work or post-graduate degree in a relevant field with 2+ years of extensive crisis experience, or a BSW with 5+ years of relevant experience.
  • Registration in good standing with the Ontario College of Social Workers and Social Service Workers or another relevant regulatory body in Ontario.
  • Proven experience in crisis support and working with complex senior populations.
Preferred Skills
  • Experience in crisis intervention.
  • Ability to support older adults, including those with significant mental health and/or addiction issues, trauma histories, cognitive impairments, and chronic health conditions.
  • Proficiency in case management, service planning, and therapeutic counseling.
  • Ability to collaborate effectively within a multi-disciplinary team.
  • Knowledge of addiction theories, evidence-based treatment models, and therapeutic modalities.
  • Strong understanding of issues affecting older adults and geriatric populations.
  • Excellent assessment, clinical, and intervention skills with a client-centered approach.
  • Strong communication, documentation, time management, and organizational skills.
  • Commitment to anti-oppression, anti-racism, trauma-informed, and harm reduction principles.
  • Self-directed, independent, and flexible with sound judgment.
  • Ability to minimize barriers for low-income individuals with mental health and substance use challenges.
  • Familiarity with issues affecting marginalized communities and relevant policies.
  • Competency in computer skills and client information systems.
  • Proficiency in a second language is a valuable asset.
