Job Overview

Hamilton Health Sciences is seeking a skilled Registered Practical Nurse to join our team as a Care Coordinator for geriatric patients in our rehabilitation department. As a member of our interdisciplinary healthcare team, you will play a crucial role in assessing and providing care to meet the unique needs of our senior patients suffering from orthopedic conditions.

About the Role

In this rewarding position, you will work collaboratively with registered nurses and other healthcare professionals to develop and implement personalized care plans that optimize the functional goals of our patients. Your expertise in rehabilitation nursing, combined with your strong communication skills, will enable you to effectively manage patient care and provide education to patients and their families.

Responsibilities

Your key responsibilities will include:

• Conducting comprehensive assessments of patients to identify their individual needs and develop targeted care plans

• Collaborating with healthcare teams to implement care plans and monitor patient progress

• Providing direct patient care, including wound management, pain management, and enteral/parenteral nutrition

• Educating patients and their families on disease management, medication use, and self-care techniques

Qualifications

To be successful in this role, you will need to possess a current Certificate of Registration with the College of Nurses of Ontario as a Registered Practical Nurse. You should also have demonstrated knowledge of rehabilitation nursing, outcome measurement, and a primary nursing model of care. Additionally, you must have excellent communication, teamwork, problem-solving, and interpersonal skills.

Salary Information

The salary for this position is estimated to be around $95,000 per year, based on industry standards and the level of experience required. This figure is subject to change and may vary depending on various factors.
