We are seeking a Medical Care Coordinator for our Short Stay Medicine Unit at Hamilton General Hospital. As a member of our interdisciplinary team, you will play a critical role in coordinating and evaluating the nursing care of patients with multisystem medical conditions.

The ideal candidate will possess strong leadership skills, excellent communication and interpersonal skills, and the ability to work effectively in a fast-paced environment. You will be responsible for collaborating with patients, families, and healthcare teams to develop and implement personalized care plans, ensuring seamless transitions to community settings.

Job Requirements:

  • Current Registration with the College of Nurses of Ontario as a Registered Nurse
  • Current Basic Cardiac Life Support (BCLS) certification
  • HHS' Critical Practice Orientation (CPO) Telemetry education and experience would be an asset
  • Minimum one (1) year nursing experience in a medical setting within the last 2 years preferred

Working Conditions:

This is a part-time position with rotating days, evenings, nights, weekends, and holidays. The salary range for this position is $39.07 - $56.00 per hour.
