About the Role

We are seeking a skilled and compassionate Registered Nurse to join our dynamic team at SE Health. As a Patient Care Coordinator, you will play a vital role in delivering high-quality patient care and supporting our clients' overall well-being.

Job Summary

The successful candidate will work closely with our interdisciplinary team to provide comprehensive nursing care to clients in our clinic setting. This includes assessing client needs, developing and implementing individualized care plans, and collaborating with healthcare professionals to ensure seamless care coordination.

Key Responsibilities
  • Assess client needs and develop individualized care plans to promote optimal health outcomes
  • Provide direct patient care, including wound care, medication administration, and vital sign monitoring
  • Collaborate with healthcare professionals to ensure continuity of care and effective communication
  • Maintain accurate and up-to-date records of client progress and care provided
  • Participate in quality improvement initiatives to enhance patient care and services

Requirements

  • Current Certificate of Competence with the College of Nurses of Ontario
  • Availability to work 12-hour shifts, including every other weekend
  • Updated immunization records, clear criminal background check, and Vulnerable sector check required upon hire
  • A willingness to attain required SE Health certifications
  • Strong computer skills in MS Word, MS Excel, Email, etc.
  • Able to work independently as well as part of a team
  • Well-developed interpersonal skills
  • Excellent organizational skills
  • Effective problem-solving and conflict management skills
  • Works well in a changing environment
  • A demonstrated appreciation for Quality Improvement processes
  • A current CPR certificate is required

Working Environment

This is a physically demanding position that requires sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level (i.e. shoulder flexion), and numerous hand grips of varying forces.
