Career Opportunity: We are seeking a highly skilled Registered Nurse to join our team as a Medical Care Coordinator.

Job Summary

The Medical Care Coordinator provides leadership in the coordination and evaluation of nursing care for patients with multisystem medical conditions awaiting transition to the community setting.

This role involves working collaboratively with an interdisciplinary team to coordinate care and education of patients and their families. The successful candidate will have strong leadership and communication skills, with the ability to work independently and collaboratively within a fast-paced environment.

Key Responsibilities

  • Coordinate and evaluate nursing care for patients with complex medical needs
  • Develop and implement education plans for patients and their families
  • Collaborate with the interdisciplinary team to ensure seamless care transitions
  • Provide leadership and guidance to junior staff members
  • Maintain accurate and up-to-date patient records

Qualifications

  • Current registration with the College of Nurses of Ontario as a Registered Nurse
  • Basic Cardiac Life Support (BCLS) certification
  • Experience in a medical setting, preferably with geriatric patients
  • Knowledge of current nursing assessment and management techniques
  • Strong leadership and communication skills

Working Conditions

This role is a part-time position with rotating shifts, including days, evenings, nights, weekends, and holidays. The salary range for this position is $39.07 - $56.00 per hour, depending on experience.
