Primary Care Nurse with Chronic Disease Management Experience

We are seeking a dedicated Registered Nurse with expertise in primary care nursing and chronic disease management to join our team at Northern Health Authority.

Job ResponsibilitiesAs a Primary Care Nurse, you will provide direct clinical care and services to individuals, groups, and communities. Your experience in chronic disease management settings will be invaluable in developing and implementing care plans that address the unique needs of our patients.
Key Qualifications- Registration with BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings, including experience developing and providing formal education to patients, families, the general public, or an equivalent combination of training and experience.
