About the Role:

We are seeking a highly skilled Community Health Nurse to join our team in providing exceptional geriatric care to patients and their families. As a key member of our community-based healthcare services, you will play a vital role in promoting health, preventing illness, and supporting patients to live confidently and safely at home.

Key Responsibilities:

  • Conduct comprehensive assessments of clients' needs and develop personalized care plans to ensure optimal health outcomes.
  • Provide direct client care, including coaching, interventions, and counseling, to support clients with complex chronic conditions.
  • Collaborate with interprofessional teams, primary care providers, and community resources to facilitate timely and appropriate health services.
  • Develop and implement short- and long-term care plans, monitoring progress and adjusting as needed to achieve desired outcomes.
  • Promote client-focused care, respecting clients' dignity and autonomy, while advocating for their needs and preferences.

Requirements:

  • Current practicing registration as a Licensed Practical Nurse (LPN) with the British Columbia College of Nurses and Midwives (BCCNM).
  • One year's recent experience in assessing and treating complex geriatric and adult patients with chronic health conditions in an acute or community/outpatient care setting.
  • Demonstrated knowledge of gerontology, adults living with complex frailty, and chronic illnesses, as well as competence in care management principles.
  • Excellent communication, problem-solving, and interpersonal skills, with the ability to work effectively in a dynamic environment.
