SalaryThe salary for this position ranges from $41.42 to $55.91 per hour, with opportunities for loan forgiveness programs offered through the Federal and BC Government for eligible professions.
Job DescriptionThis role involves providing clinical follow-up according to patients' primary care plans and personal goals of care, developing collaborative plans with interprofessional teams, and delivering direct clinical care and services to individuals, groups, and communities.
In partnership with local community providers, we are working together to redesign the primary and community care system so that it works better for patients and healthcare providers.
Required Skills and Qualifications- Registration with BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings.
- Current valid Driver's License.
- Comprehensive benefit packages including extended health/dental and a municipal pension plan for part-time and full-time employees.
- Four weeks vacation with one year of continuous service.
- Financial Support for Moving Expenses is available for eligible positions.
