Job Summary

Fraser Health is seeking a skilled Registered Nurse to join our team in a Registered Nurse, Outreach role.


Responsibilities

  1. Assess patients and collect information from various sources using skills of observation, communication, data review, and physical assessment.
  2. Makes a nursing diagnosis of the patient's condition and determines whether the condition can be improved or resolved by an appropriate nursing intervention.
  3. Plans, performs, and evaluates nursing interventions and care.
  4. Provides support in the designated clinical area with the implementation of patient care plans and protocols requiring specific nursing skills.
  5. Participates in the interprofessional decision process to stabilize patient care.
  6. Acts as a role model for nurses by providing collegial sharing of information and demonstrating advanced practice skills.
  7. Establishes a therapeutic relationship with the patient by demonstrating empathy, trust, and respect.
  8. Advocates for the patient's rights to privacy, dignity, and access to information.
  9. Coordinates nursing care for the patient, sets priorities, assigns and delegates tasks, and makes referrals to the interprofessional care team.
  10. Collaborates with members of the interprofessional care team to identify and resolve issues.
  11. Plans and discusses referral information with patients and their families/caregivers.
  12. Documents assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals.
  13. Participates in research and quality improvement activities.
  14. Participates in and attends in-services and other educational programs.
  15. Acts as a preceptor or mentor to assigned new nursing staff and students.
  16. Provides feedback to the Manager/delegate regarding the effectiveness of patient care programs and policy/clinical decision support tools and equipment.
  17. Participates on nursing committees, care conferences, and councils.

Qualifications

  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two years' recent related acute care experience and completion of a nursing program in the designated clinical area (critical care or emergency nursing), or an equivalent combination of education, training, and experience.
  • Current certification in Basic Cardiac Life Support (BCLS) or Advanced Cardiac Life Support (ACLS).

Skills and Abilities

  • Comprehensive knowledge of nursing theory and evidence-based practice within the designated clinical area.
  • Ability to role model evidenced-based advanced practice skills in the designated clinical area.
  • Ability to complete initial and ongoing patient assessment (clinical and diagnostic reasoning) and provide nursing care.
  • Ability to communicate effectively with patients, families/caregivers, the public, medical staff, and members of the interprofessional care team.
  • Ability to promote person-centered care that demonstrates empathy for patients and families/caregivers, sensitivity to diverse cultures and preferences, and patient advocacy.
  • Ability to integrate and evaluate data from multiple sources to problem-solve effectively.
  • Ability to teach and instruct patients and families/caregivers about topics essential to their health care and well-being.
  • Ability to organize and prioritize work.
  • Ability to collaborate with and promote cooperation among members of the interprofessional care team.
  • Ability to use factual information, prior learning, and basic principles and procedures to support nursing decisions and actions with relevant research-based evidence/evidence-informed practice.
  • Ability to operate related equipment, including applicable computer applications.
  • Physical ability to perform the duties of the position.
