About the Role

We are seeking a skilled and compassionate Geriatric Nurse Specialist to join our team at Fraser Health. This is an exciting opportunity to work with patients in a specialized seniors clinic setting, providing high-quality care and support.

Job Summary

The Registered Nurse provides direct patient care by assessing, planning, providing and evaluating nursing care. This includes teaching patients and families about geriatric health issues, illness management, prevention and treatment. The ideal candidate will have two years of recent related acute care experience working with a geriatric population.

Key Responsibilities
  • Assess patients referred to a specialized seniors clinic (SSC) and collects information from various sources using skills of observation, communication, data review, patient information received from referring healthcare providers, and physical assessment.
  • Makes a nursing diagnosis of the patient's condition and determines whether the condition can be improved or resolved by an appropriate nursing intervention.
  • Plans, performs and evaluates nursing interventions and care.
  • Collaborates with patients and their families/caregivers in developing the care plan and encourages family/caregiver participation in the development, implementation and ongoing modification of the care plan.
Requirements
  • Graduation from an approved school of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two (2) years of recent related acute care experience working with a geriatric population, or an equivalent combination of education, training and experience.
