POSITION SUMMARY

The Clinical Nutrition Specialist at Prairie Mountain Health is tasked with delivering comprehensive nutritional support and guidance to patients, residents, and clients. This role involves active participation in interdisciplinary healthcare teams, providing instruction to nutrition services personnel, and aiding in the formulation of programs, menus, policies, and educational resources pertinent to nutritional care. Additionally, the specialist engages in strategic planning and ongoing quality enhancement initiatives.

RESPONSIBILITIES:

Overview:

Collaborating with healthcare teams, including patients, residents, clients, and their families, while adhering to current best practices and established standards:

  • Conducts thorough nutrition assessments.
  • Develops, monitors, and updates personalized nutrition care plans.
  • Delivers nutrition education tailored to individual needs.
  • Maintains accurate records in accordance with regional and program standards.
  • Provides specialized nutrition guidance for program-specific procedures.
  • Engages in multidisciplinary team meetings, including Care Conferences, Clinical Rounds, and Discharge Planning.

As a member of the Prairie Mountain Health Clinical Dietitian team:

  • Participates in team meetings to contribute to the development of program action plans and nutrition-related policies.
  • Involves in sub-committees as necessary.
  • Reports on pertinent issues from clinical practice areas.
  • Shares expertise and knowledge with team members.
  • Prepares statistical and status reports for management review.
  • Undertakes additional duties as assigned.
