POSITION SUMMARY

As a Mental Health Support Specialist, you will provide vital mental health services to residents of Prairie Mountain Health. You'll work in both community and facility settings, collaborating with interdisciplinary teams to deliver comprehensive care.

Your role is crucial in supporting individuals on their personal recovery journey. You'll empower clients and families by focusing on strengths-based approaches, promoting independence, self-responsibility, and well-being within their communities.

RESPONSIBILITIES

  • Build trusting relationships with clients and families, guiding them through case formulation, intervention planning, and service development.
  • Conduct thorough initial clinical and psychosocial assessments, incorporating collateral information to create a comprehensive understanding of each client's needs.
  • Identify client strengths, skills, resources, and support systems to develop personalized recovery plans.
  • Assess risks for self-harm, violence, or mental health deterioration, intervening effectively in crisis situations.
  • Provide evidence-based counseling services to clients and families, utilizing a range of therapeutic modalities.
  • Collaborate with clients to identify essential services and connect them with appropriate resources and agencies.
  • Advocate for clients' needs, ensuring they receive the necessary support and care.
  • Engage family members as active participants in care planning, providing emotional support and education while respecting privacy legislation.
  • Coordinate comprehensive care planning meetings involving families, healthcare professionals, and community agencies to facilitate a cohesive and integrated service system.
  • Document assessments, interventions, and outcomes accurately and professionally, adhering to established standards.
  • Contribute to service improvement initiatives by identifying gaps in care and advocating for innovative solutions.
