POSITION SUMMARY

The Clinical Nutrition Specialist is tasked with delivering comprehensive nutritional care and guidance for patients, residents, and clients. This role involves active participation in healthcare and dietetic teams, providing instruction on nutrition care plans to nutrition services personnel, and contributing to the formulation of programs, menus, policies, procedures, and educational resources related to nutritional care. The specialist also engages in strategic planning and ongoing quality enhancement initiatives.

RESPONSIBILITIES:

Overview:

In collaboration with healthcare teams, including patients, residents, clients, and/or their families; utilizing current best practice evidence; and adhering to established standards of practice:

  • Conducts thorough nutrition assessments.
  • Develops, monitors, and updates nutrition care plans.
  • Delivers nutrition education to clients and staff.
  • Maintains records in accordance with regional/program standards.
  • Provides nutritional expertise for specific program-related procedures.
  • Engages in multi-disciplinary team meetings (Care Conferences, Clinical Rounds, and Discharge Planning).

As a member of the Prairie Mountain Health Clinical Dietitian team:

  • Participates in team meetings, including the creation of program action plans and nutrition care-related policies and procedures.
  • Involves in sub-committees as necessary.
  • Reports on pertinent issues from clinical areas of responsibility.
  • Shares knowledge and expertise with team members.
  • Prepares statistical and status reports for management.
  • Performs additional duties as assigned.
