We are currently seeking a compassionate and experienced Social Worker to join our interdisciplinary team within the Medicine Program. The ideal candidate will play a vital role in supporting patients and their families throughout their healthcare journey.
Job SummaryAs a Patient Care Social Worker, you will provide comprehensive support and discharge planning services to patients facing complex medical situations. Your responsibilities will encompass conducting thorough psychosocial assessments, offering emotional support, and developing individualized discharge plans that ensure seamless transitions back home or to other care settings.
Primary Responsibilities- Conduct comprehensive psychosocial assessments to understand patient needs and challenges.
- Develop and implement individualized discharge plans in collaboration with patients, families, and the healthcare team.
- Provide emotional support and counseling to patients and their caregivers during challenging times.
- Advocate for patients' rights and well-being throughout their care journey.
- Collaborate effectively with physicians, nurses, and other healthcare professionals to ensure coordinated and holistic patient care.
- Stay informed about relevant legislative acts and policies impacting patient care.
- Master's Degree in Social Work from an accredited university.
- Active membership in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Minimum three years of relevant social work experience within an acute care setting preferred.
- Demonstrated knowledge of legislative acts, including the Mental Health Act and Long Term Care Homes Act.
- Exceptional communication, interpersonal, and problem-solving skills.
- Strong organizational and time management abilities.
