Job Summary

As a Mental Health Social Worker, you will play a vital role in providing high-quality patient care as part of our dedicated Mental Health Team. You will work collaboratively with a multidisciplinary team to assess, treat, and support individuals experiencing serious mental illness within the Adult Outpatient Mental Health program.

Key Responsibilities

  • Conduct comprehensive assessments, develop individualized treatment plans, and provide short-term counseling to clients with serious mental illness.
  • Facilitate individual and group therapy sessions, utilizing various therapeutic modalities such as Brief Solution Focused model, Cognitive Behavioral Therapy (CBT), and Dialectical Behavior Therapy (DBT).
  • Maintain accurate and detailed clinical documentation of assessments, treatment plans, and client progress.
  • Collaborate effectively with team members, the hospital community, and external agencies to ensure seamless care coordination.
  • Provide education and resources to patients, families, and the community regarding mental health issues.
  • Contribute to data collection and analysis to support program evaluation and improvement.

Qualifications

  • Master's degree in Social Work (MSW) from an accredited institution.
  • Three to five years of experience working with clients experiencing serious mental illness.
  • Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Advanced counseling and assessment skills, including proficiency in various short-term therapeutic modalities.
  • Demonstrated ability to work collaboratively within a multidisciplinary team environment and independently.
  • Excellent communication, interpersonal, and problem-solving skills.
