Position Overview
We are in search of a Social Worker to become an integral part of our interdisciplinary team. This position entails offering support and planning services for patient discharges, ensuring that patients and their families receive the necessary assistance. The Social Worker will perform psychosocial evaluations, provide emotional support, and develop detailed discharge plans for intricate cases.
Key Responsibilities
The successful candidate will possess a Master's Degree in Social Work and have at least three years of pertinent experience in an acute care environment. It is essential to be a member in good standing with the Ontario College of Social Workers and to demonstrate strong communication and interpersonal abilities. Furthermore, familiarity with legislative frameworks and experience in care transition planning are critical for this role.
Required Qualifications
  • Master's Degree in Social Work from an accredited institution is mandatory
  • Active membership with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
  • At least three years of relevant experience in social work or discharge planning within an acute care context is preferred
  • Understanding of legislative frameworks such as the Mental Health Act and Long Term Care Homes Act
  • Proven capability in conducting assessments, treatment planning, conflict resolution, and problem-solving
  • Excellent communication and interpersonal skills
  • Strong organizational and coordination abilities
  • Adaptability to meet additional responsibilities as program needs evolve
