**Job Summary**

We are seeking a skilled Social Worker to join our team. The ideal candidate will have a Master's degree in Social Work from an accredited university and previous experience in healthcare, particularly in cardiovascular disease management and cardiac rehabilitation.

**Key Responsibilities**

• Provide social work services to patients with cardiovascular disease

• Collaborate with healthcare teams to develop and implement care plans

• Conduct assessments and provide counseling to patients and their families

• Develop and maintain relationships with community resources and healthcare providers

• Participate in quality improvement initiatives and contribute to a culture of excellence

**Requirements**

• Master's degree in Social Work from an accredited university

• Previous experience in healthcare, particularly in cardiovascular disease management and cardiac rehabilitation

• Membership in the Ontario College of Social Workers and Social Service Workers
