About the Role

We are seeking a skilled Social Worker to join our team at Southlake Regional Health Centre. As a key member of our inter-professional Medicine team, you will provide assessment, treatment, consultation, and education services to patients and their families.

Key Responsibilities

  • Provide short-term crisis and grief counseling for patients, families, and team members.
  • Counsel patients and families regarding hospital admission and discharge to ensure continuity of care.
  • Maintain documentation of clinical assessments, treatment plans, and patient clinical progress.
  • Act as a resource and liaison to community stakeholders and facilitate referrals to community resources.
  • Develop and maintain relationships with community agencies and organizations to support patient care.
  • Collaborate with healthcare providers to ensure comprehensive patient care.
  • Participate in ongoing learning and professional development to enhance skills and knowledge.

Requirements

  • Masters of Social Work from an accredited university.
  • Membership in the Ontario College of Social Workers and Social Service Workers required.
  • Two years' experience as a social worker in an acute care setting.
  • Excellent written, verbal, and telephone communication skills.
  • Demonstrated ability to work independently and as a strong team player.
  • Cultural sensitivity and diversity.
  • Knowledge of governmental financial programs and resources.
