About the Role
The Social Worker works as a member of the interdisciplinary team to provide social work support and discharge planning services and expertise. The Social Worker provides in-depth psychosocial assessments and appropriate interventions for patients and their caregivers, including resource education, emotional support, counselling, case management, and advocacy, to maximize patient quality of life within their community.
Key Responsibilities
- Provide comprehensive psychosocial assessments and develop individualized treatment plans for patients and their caregivers.
- Collaborate with the interprofessional healthcare team to coordinate comprehensive discharge plans for complex patients and their families.
- Provide education and support to patients and their caregivers on community resources and system navigation.
- Develop and maintain relationships with community agencies and organizations to ensure seamless transitions in care.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
- Masters' Degree in Social Work from an accredited university.
- Member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Minimum three years relevant social work/discharge planning experience within an acute care setting.
- Demonstrated ability to provide assessments and treatment planning, conflict resolution, and problem-solving for both inpatients and outpatients.
- Working knowledge of legislative acts, including the Mental Health Act, Long Term Care Homes Act, and Health Care Consent Act.
- Excellent communication and interpersonal skills, with the ability to work collaboratively with patients/families, interprofessional team members, and other stakeholders.
- Strong organizational and coordination skills, with the ability to prioritize multiple competing workload demands.
