Position Overview
We are in search of a qualified Social Worker to become a vital part of our interdisciplinary team. This position entails offering support and planning services for patient discharge, ensuring a smooth transition for patients and their families. The Social Worker will perform comprehensive psychosocial evaluations, deliver emotional assistance, and develop detailed discharge strategies for intricate cases.
Key Responsibilities
The successful candidate should possess a Master's Degree in Social Work and have at least three years of pertinent experience in an acute care environment. Membership in good standing with the Ontario College of Social Workers is required, along with strong communication and interpersonal abilities. Familiarity with relevant legislative frameworks and experience in care transition planning are critical for this role.
Essential Qualifications
  • Master's Degree in Social Work from an accredited institution is mandatory
  • Active membership with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
  • Minimum of three years of relevant experience in social work or discharge planning within an acute care context is preferred
  • Knowledge of legislative frameworks, including the Mental Health Act and Long Term Care Homes Act
  • Proven ability to conduct assessments, treatment planning, conflict resolution, and problem-solving
  • Excellent communication and interpersonal skills
  • Strong organizational and coordination capabilities
  • Adaptability to meet additional responsibilities as program needs evolve
