Job Requirements

  • Masters of Social Work from an accredited university
  • Previous experience relating to patients with cardiovascular disease is an asset
  • Previous experience in Chronic Disease Management is an asset
  • Previous experience in Cardiac Rehabilitation setting is an asset
  • Membership in the Ontario College of Social Workers and Social Service Workers
