Job SummaryWe are seeking a highly motivated and organized Case Manager to join our team. The successful candidate will be responsible for providing case management services to clients, including assessment, planning, and coordination of services.
Key Responsibilities- Assessment and Planning: Conduct thorough assessments of clients' needs and develop individualized plans to address their goals and objectives.
- Service Coordination: Coordinate and connect clients with community resources and services to support their well-being and independence.
- Case Management: Provide ongoing case management services to clients, including regular check-ins and progress monitoring.
- Collaboration: Work collaboratively with internal and external stakeholders, including healthcare providers, social workers, and community agencies.
- Education: Bachelor's degree in a related field, such as social work or a related field.
- Experience: 2-3 years of experience in case management or a related field.
- Skills: Excellent communication and interpersonal skills, with the ability to work with diverse populations.
