The Bilingual Neurology Nurse Case Manager plays a pivotal role in delivering exceptional patient care through a collaborative approach. This position involves continuous assessment, planning, and coordination of services to meet the needs of patients and healthcare providers.
Key Responsibilities:
- Manage a caseload of patients and liaise with physicians and clinics.
- Conduct home-based work with significant travel requirements within the designated territory.
- Serve as the primary contact for patients, healthcare professionals, and vendors.
- Facilitate patient progress from enrollment through therapy.
- Maintain comprehensive documentation to ensure continuity of care.
- Provide updates on reimbursement processes and patient training needs.
- Report any adverse events or product complaints promptly.
- Enhance communication and coordination among medical professionals, patients, and internal departments.
- Deliver educational sessions in various settings, including clinics and homes.
- Collaborate with pharmaceutical representatives to optimize patient outcomes.
- Uphold patient confidentiality and advocate for positive health outcomes.
- Commit to continuous quality improvement and professional development.
Qualifications:
- Registered Nurse with an active license in the relevant province; a BScN is preferred.
- Experience in neurology, particularly with conditions like Parkinson's disease, is advantageous.
- Strong understanding of the reimbursement landscape.
- Recent involvement with Patient Support Programs.
- Fluency in both French and English, with excellent communication skills.
- A minimum of five years of direct clinical experience.
- Proficient in Microsoft Office and web-based applications.
- Valid driver's license and access to a reliable vehicle.
