About the Role

S.U.C.C.E.S.S. is seeking a skilled Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to clients, helping them to achieve their goals and improve their overall well-being.

Key Responsibilities
  • Client Assessment and Planning: Conduct thorough assessments of clients' needs and develop personalized plans to address their challenges.
  • Case Management: Provide ongoing support and guidance to clients, helping them to access resources and services that meet their needs.
  • Community Outreach: Build relationships with community partners and stakeholders to promote S.U.C.C.E.S.S.'s services and programs.
  • Program Development: Contribute to the development and implementation of new programs and services that meet the evolving needs of our clients.
Requirements
  • Education: Bachelor's degree in a related field, such as social work or community development.
  • Experience: 2 years to less than 3 years of experience in a related field, such as case management or community outreach.
  • Skills: Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Language: Fluency in English is required.
  • Work Hours: 35 hours per week.
