Position Overview:
The Bilingual Neurology Nurse Field Case Manager plays a pivotal role in the collaborative process of assessing, planning, facilitating, coordinating services, evaluating, and advocating for patients and healthcare providers. This position is dedicated to delivering exceptional customer service and ensuring quality treatment outcomes through effective communication and resource management.
Key Responsibilities:
- Manage a caseload of patients and healthcare providers, ensuring seamless communication and support.
- Work remotely with significant travel within the designated territory, including occasional overnight stays.
- Serve as the primary contact for patients, physicians, healthcare professionals, and vendors.
- Coordinate and facilitate patient progress from enrollment through therapy.
- Maintain thorough documentation in patient files to ensure continuity of care and identify any changes in treatment.
- Provide updates on reimbursement status, patient training needs, and pharmacy-related requests.
- Report any adverse events or product complaints promptly, adhering to established policies.
- Enhance communication and coordination among medical professionals, patients, and internal departments.
- Deliver in-clinic, in-home, and hospital training while fostering relationships on behalf of the program.
- Conduct both face-to-face and virtual education sessions for patients and their support networks.
- Collaborate effectively with pharmaceutical representatives.
- Uphold patient confidentiality and advocate for positive outcomes, addressing ethical issues as per professional standards.
- Commit to continuous quality improvement and professional development.
Qualifications:
- Registered Nurse with an active, unrestricted license in the province of employment; a BScN is preferred, though relevant experience may be considered.
- Experience in neurology, particularly with conditions such as Parkinson's disease, is advantageous.
- Proficient understanding of the reimbursement landscape and patient support programs.
- Fluency in both French and English, with strong communication skills.
- A minimum of five years of direct clinical experience.
- Strong organizational and critical thinking abilities, with a focus on case management.
- Proficient in Microsoft Office applications and web-based platforms.
- Valid driver's license and access to a reliable vehicle.
