Job Title: Quality Improvement Lead

We are seeking a highly skilled and experienced Quality Improvement Lead to join our team at Hawthorne Place Care Centre. As a Quality Improvement Lead, you will play a critical role in ensuring the highest standards of quality and safety in our care centre.

Key Responsibilities:
  • Coordinate Quality Improvement Program: Develop and implement a comprehensive quality improvement program that aligns with the care centre's strategic objectives.
  • Lead Quality Team Meetings: Facilitate regular quality team meetings to discuss quality improvement initiatives, share best practices, and address any quality-related concerns.
  • Resident Quality Inspection (RQI) Process: Coordinate the RQI process, including training staff on the inspection guides, interview questions, and surveys.
  • Quality and Risk Management: Ensure that all quality management and risk management-related activities are reviewed, discussed, and follow-up actions identified as necessary.
  • Indicators and Data Analysis: Monitor indicators on the public website of HQO and Your Health System, and communicate data analysis, trends, and quality improvement activities to residents and family council.
  • Accreditation and Compliance: Work with the Accreditation Coordinator to ensure all quality and safety standards for CARF accreditation are being addressed.
Requirements:
  • Comprehensive Knowledge: Comprehensive knowledge of healthcare practices, trends, and issues in long-term care and impacts on other healthcare sectors/community at large.
  • Proven Management Experience: Proven management experience in LTC, with a strong track record of quality improvement and risk management.
  • Excellent Communication Skills: Excellent written and oral communication skills, with the ability to effectively communicate with residents, families, and staff.
  • Quality/Risk Management: Excellent knowledge of Quality/Risk Management philosophies and programs, with the ability to apply this knowledge in a practical setting.
  • Master Trained in RQI: Master trained in the Resident Quality Inspection process, with the ability to train and coach staff on this process.
  • Computer Applications: Working knowledge of relevant computer applications and applicable legislation.
  • Positive Attitude: Positive working attitude, with a strong commitment to teamwork and a focus on people's feelings when interacting.
  • Supervisory References: Two supervisory references required, with a strong emphasis on quality improvement and risk management experience.
