We are in search of a qualified individual to become a vital member of our innovative Rehabilitation Team as a Rehabilitation Specialist. Our therapy and rehabilitation program is expanding, and we invite professionals who are eager to contribute to this exciting development.

This is a Part-Time role with variable hours.

Responsibilities of the Role

Our rehabilitation specialists play a crucial role in delivering high-quality patient care.

  • Conduct professional assessments of patients in community settings and their residences
  • Develop and uphold effective Care Plans in alignment with Community Health Services, either independently or in collaboration with Home and Community Care Support Services (HCCSS)
  • Collaborate with patients to establish appropriate, measurable goals and outcomes in conjunction with the care team
  • Operate independently while also being an integral part of an interdisciplinary care team
  • Provide active guidance regarding workplace Health and Safety
  • Perform additional duties as necessary

Qualifications Required

  • Master’s Degree (or equivalent) in Physiotherapy from an accredited institution
  • Registration with the College of Physiotherapists of Ontario
  • Proficient computer skills, including email, word processing, and scheduling systems
  • Strong assessment capabilities and the ability to engage in effective treatment programs
  • Deliver one-on-one therapeutic support to clients
  • Encourage client independence both at home and in the community
  • A reliable vehicle for regular travel
  • Excellent verbal and written communication skills in English
  • Proficiency in French is an asset
  • Clear Background and Vulnerable Sector Check
