Position Overview

The Licensed Practical Nurse (LPN) in the Hemodialysis Unit plays a crucial role in delivering nursing care within a specialized nephrology environment. Reporting to the Patient Care Manager, the LPN collaborates with an interdisciplinary team to perform a variety of complex renal interventions. This position requires the LPN to operate independently while recognizing when to engage Registered Nurses (RNs) for collaborative support in high-acuity situations.

Key Responsibilities

  • Conduct assessments, develop care plans, implement interventions, and evaluate patient outcomes during pre-dialysis, dialysis, and post-dialysis phases.
  • Communicate effectively with patients and their families, providing education and emotional support throughout the treatment process.
  • Perform essential nursing duties related to hemodialysis, including patient admission assessments, monitoring vital signs, and managing dialysis equipment.
  • Collaborate with RNs and other nephrology team members to ensure comprehensive patient care and address any emerging issues.
  • Maintain accurate documentation of patient care activities and observations to ensure continuity of care.
  • Participate in team meetings and quality improvement initiatives to enhance patient care practices.
  • Support the orientation and training of new staff members, sharing knowledge and best practices.

Qualifications

Candidates must possess a diploma from an accredited Practical Nursing program and hold current registration with the relevant nursing regulatory body. A recognized Dialysis Program certification and a minimum of one year of recent experience in acute care settings are preferred. Basic cardiac life support certification is also required.

  • Strong knowledge of CPR and hemodialysis procedures.
  • Excellent verbal and written communication skills.
  • Ability to work effectively within a multidisciplinary team.
  • Proficient in prioritizing tasks and managing time efficiently.
  • Capable of operating specialized medical equipment and teaching patients about their care.

Commitment to Safety

Adherence to safety protocols is paramount. The LPN must ensure a safe environment for both patients and staff by following established guidelines and reporting any safety concerns promptly.

Vaccination Requirement

In accordance with public health mandates, all employees must be fully vaccinated against COVID-19, and proof of vaccination status will be required.
