Position Overview

We are seeking a Licensed Practical Nurse to become a vital part of our Integrated Care Team.

This role involves serving a diverse community and providing essential care to clients in various settings.

Responsibilities of the Licensed Practical Nurse include:

Collaborating across multiple facilities and services, or conducting outreach/home visits to deliver comprehensive care to primary care clients. Assisting clients and their families facing health challenges throughout the primary care spectrum. Seeking guidance from Registered Nurses (RNs), Registered Psychiatric Nurses (RPNs), Nurse Practitioners (NPs), or Physicians (MDs) when client care needs are complex. Engaging closely with the RN/RPN/NP/MD in direct care tasks, including assessment, planning, implementation, evaluation, and documentation of client care as part of a cohesive team. Building and nurturing therapeutic relationships founded on respect and understanding. Supporting clients and their families in achieving and maintaining optimal health outcomes.

Required Qualifications

Education & Experience

Completion of a recognized Practical Nurse program. Active registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM). A minimum of one (1) year of relevant experience or completion of a full clinical placement in a community setting, providing care through a holistic approach, or an equivalent combination of education and experience. A valid BC Driver's License is preferred.

Skills & Competencies

In-depth knowledge of various healthcare disciplines and their roles in client care. Proven ability to navigate diverse situations requiring initiative, creativity, and professional judgment. Capability to assess clients at risk and familiarity with relevant legislation; including the Mental Health Act, Adult Guardianship, and Public Health Act. Experience in delivering culturally safe and competent care, particularly for Indigenous populations. Understanding of Trauma-Informed Practice, Harm Reduction, and Recovery-Oriented Care. Adherence to the standards set by the BCCNM for Licensed Practical Nurses. Broad knowledge of working with clients and families across different life stages. Familiarity with community resources and policies. Demonstrated decision-making and analytical skills. Ability to work effectively as part of an interdisciplinary team. Strong verbal and written communication skills. Proficiency in managing conflict situations effectively. Organizational skills to plan and prioritize work efficiently. Physical capability to fulfill the duties of the position. Basic computer skills, including familiarity with programs like Word and Microsoft Outlook. Competence in clinical techniques and the use of necessary equipment and supplies. Proficiency in CPR techniques. Ability to effectively utilize a computerized client care information system.
