Position Overview

Trillium Health Partners is seeking a highly skilled and compassionate Regional Indigenous Navigator to join our team. As a key member of our Regional Cancer Program, you will play a vital role in providing support and advocacy for Indigenous patients and families navigating the cancer care system.

Key Responsibilities

  • Establish and maintain relationships with regional Indigenous and non-Indigenous partners to ensure seamless access to cancer services.
  • Work collaboratively with healthcare professionals to implement culturally sensitive care practices and address the unique needs of Indigenous patients.
  • Develop and maintain a regional approach to address the cancer needs of Indigenous peoples, including prevention, screening, diagnosis, treatment, and follow-up care.
  • Support the development of policies and guidelines to ensure accurate and current resources are available to support Indigenous care needs.
  • Engage with spiritual care services and equity departments to support knowledge transfer and cultural awareness.
  • Advocate on behalf of patients, families, and caregivers to ensure timely access to palliative and supportive care.
  • Document patient stories and provide patient perspective for regional Patient and Family Advisory Tables.
  • Support alignment of regional approaches with provincial quality initiatives in the full spectrum of cancer care.

Qualifications

  • Undergraduate degree in health or social sciences field; Registered Health Professional designation preferred.
  • Knowledge of the cancer care pathway as it relates to Indigenous peoples, an asset.
  • Extensive knowledge of Indigenous people's language, culture, customs, traditions, and health/socioeconomic needs.
  • Highly developed skills in culturally relevant assessment and trauma-informed therapeutic intervention.
  • Strong insight into the importance of Reconciliation with Indigenous peoples.
  • Ability to navigate the Health Canada First Nation and Inuit Health Branch Non-Insured Health Benefits Program.
  • Fluency in one or more Indigenous languages is considered an asset.
  • Demonstrated leadership, excellent communication, and problem-solving skills.
