Position Overview

Reporting to the Executive Director, the Community Support Specialist plays a vital role in delivering social support and counseling services to residents and their families within the long-term care home, as well as seniors in the broader community. Utilizing professional case management practices, this position involves conducting intake assessments, coordinating services, making referrals, facilitating case conferences, and monitoring cases.

Required Qualifications

  • Bachelor’s degree in Social Work, with a Registered Social Worker designation in Ontario
  • At least 2 years of experience in counseling or group facilitation
  • Proficient oral and written communication skills in English and Chinese; knowledge of South Asian languages is an asset
  • Successful completion of a Police Check (with vulnerable sector screening)
